If your child is currently attending another school, please complete the following section. Beginning with the current school, please list all schools the student has attended.

If baptized, please provide a copy of baptism records to the school's secretary if your child was baptized somewhere other than Sacred Heart or St. Mary's in Aberdeen.

• Certificate of immunization
• Baptism record
• Records transfer request form
• $50 registration fee (one time only, nonrefundable)
